Microlift in Two Stages

Mary Esses presented with complaints mainly in the central facial region, near the nose, but had significant concerns regarding facial surgery scars. Mindful of this, she initially opted for a less comprehensive treatment, performing the surgery in stages.

In the first surgery, performed in 2023, an Upper Face Deep Plane Microlift was done. Through hidden incisions within the hair, near the sideburn, tissues were released towards the nasolabial fold, allowing for superior repositioning of tissues and elevation of the central face.

Additionally, we made another access point in the upper scalp to release the lateral temporal region and elevate the eyebrow tail, aiding in a lighter gaze. At this time, Mary also underwent blepharoplasty, for adjustment of excess skin and fat pads around the eyes, seeking a more rested look.

Two years later, Mary returned, wishing to address the laxity that still bothered her in the neck. One advantage of the Deep Plane Microlift is that it allows for modulated interventions without needing large incisions in front of the ear or in the frontal hairline. Thus, it was possible to perform the second stage of the surgery, limited to the lower face and neck.

In this procedure, access was made behind the earlobe, allowing for the release of deep tissues in the cervical region, jaw angle, 'jowl' area, and marionette lines. These tissues were lifted and fixed behind the ear, seeking to better define the contour of the neck, jawline, and lower third of the face.

With this two-phase approach, Mary was able to progressively treat the areas that bothered her, respecting her initial desire to avoid extensive scars, and achieved a complete rejuvenation with naturalness and harmony.
